What is the Give Back a Smile program?

It is an opportunity to provide free orthodontic treatment to a child whose family cannot afford care.

This is a charitable initiative to help families who may not have the means to afford orthodontic treatment – by offering one child each year the chance to receive free orthodontic care. Dr. Monica Gawlik is proud to give back to our community and provide a child the chance to smile with confidence.

What Does the Program Include?

The selected child will receive orthodontic care at no cost, including:

  • A full orthodontic exam and personalized treatment plan
  • Orthodontic records, including x-rays
  • Braces, aligners and/or any appliances as recommended by Dr. Gawlik
  • Adjustment appointments over the course of treatment
  • One set of retainers

Who Can Apply?

We welcome applications from families who:

  • Have a child from 7-18 years of age
  • Live in our local community or nearby area
  • Are experiencing financial challenges and cannot afford orthodontic care
  • Have qualified for free dental care under the Canadian Dental Care Plan (CDCP)
  • Do not have any orthodontic coverage through an employer benefit plan
  • Be current on dental visits and have no pending dental work (ie. cleanings, fillings, crowns, etc)
  • Have a child not wearing braces currently
  • Have a child who is committed to attending their appointments and maintaining excellent oral hygiene

Each year, one deserving child will be selected.

How Can I Apply?

  1. Fill out the application form by December 31
  2. Attach your CDCP approval
